Gates' team -with Ryan- soon regrets having ignored a patient's objections to cutting up his clothes, as that releases his highly toxic ricine powder meant for revenge on officials, so they go in quarantine. Archie speaks up for everyone against rude new chief Cate Banfield's incessant micro-management. Neela loves Brenner mounting a public campaign to demand Dubenko's return, but he hesitates and womanizer Aaron dismisses her suggestion it was also a move on her. An adult student with bad burn outsmarts Sam as addict.

Abby gets a job in Boston and tries to leave County General quietly, but the staff makes it known how much they will miss her. She fights the new chief Banfield over a gunshot victim's treatment and defends Sam at a disciplinary nursing board. Abby says goodbye and leaves for Boston with Luka and their son Joe.

Dissatisfied with the interns' performance, Banfield imposes a mentoring system, with each intern and mentor giving daily feedback on the other. Brenner is paired with Tracy who stubbornly insists on doing things her way, Ryan with Morris, whom he tells to resume dating so as to coach romantically too, and Gates with Daria. Banfiled is mugged but battered less then the man, who becomes a patient after her taekwondo. A girl comes in with a broken leg and multiple old 'gym' injuries, suggesting abuse by the single mother, who denies, while her jealous sister proves the culprit. Brenner gets Lucien to cue him on Neela's taste, but accepts she refuses to date properly.

On Halloween, on dumb intern shuffles in as mermaid. Model teen Scott Raskin brings in his kid brother Danny with mild injuries after an atypical school fight over bullies insulting their single mother Annie, whom Brenner tries in vain to see reason after Scott explains she misbehaves getting drunk with his friends and even taking a teen lover- Michael Leary, who is brought in stabbed at school. Frank and Morris present a costumed surprise for Neela: Ray visits, impressing with his prosthetic legs. Sam's time is taken up by her studies for a pharmacology exam, impressing colleagues and interns but leaving Gates feeling neglected. Gates looks after kid patient Owen, who hardly talks since loosing his father, except to his parrot, which get scared; they get locked in a stairwell.

Brenner gets two Aussie mates visiting for a rugby match, wisely declines Archie hoping to learn on the match, and happily finds Neela dropping by at the cafe after treating his injured mate. Tony tires of Sam's eternal nitpicking, yet insists for her to move in, at least while her apartment is being renovated, with Alex, who seals the deal. Tony patiently gets an injured, grumpy homeless man to come in for a checkup, and finds him an amnesiac fellow war veteran. A former boxer asks Archie to help him talk his once more injured son out of boxing, for which he has no talent, basically paid to loose, to help he father pay for household needs and his own surgery. Morris systematically favors Chaz, as paramedic and Greg's brother, to other interns' due envy, ignoring warnings various experience is reserved for doctors, till they endanger a patient and Banfield must step, who is already doubly grumpy expecting a visit from her in-laws, yet s saved by Frank insisting to help with a dinner recipe and more.

Neela's brilliant intern Dr. Andrew Wade loses his first patient, and struggles to cope. When a three-year-old girl is brought in, rescued from drowning in a nearby lake, Banfield can't concentrate as it reminds her of the ultimate trauma, when she lost her only son, having assumed and assured her husband his symptoms were as minor as the previous times, losing so much time even Mark Greene's brilliant leukemia diagnose came too late.

A woman who broke her leg while trying to escape a house fire is brought in with her husband. The husband, Mr. Lotery did not let the paramedics to check him over. Then his brother-in-law arrives, saying that he predicted that something like the house fire would happen - it was actually an arson. Mr. Lotery's brother-in-law becomes aggressive and accuses him of being a pervert, specifically a pedophile. Mr. Lotery denies the accusations. He was sued but found not guilty. When Mr. Lotery is waiting for his wife who's in line for X-ray, he has the chance to calm down a young girl who is afraid of needles. When his brother-in-law sees this, he brutally assaults him, while Dr. Brenner calmly watches nearby... Meanwhile, Neela gets sued for malpractice.

As a snowstorm hits Chicago, Sam's son Alex crashes a car and ends up in critical condition in the ER. Neela gives a deposition in the malpractice suit against her. Dr. Morris and Dr. Banfield are both at a medical conference in Omaha, and when the snow cancels all flights out they share a memorable and surprising evening.

Plus, the Immigration Office wants to deport her.

The police wheel in a casualty of a drug deal-turned-shoot-out. They also bring in an injured woman who was the shooter, plus a pair of wounded brothers who were shot by the man who was shot dead. A 13-year old girl with sickle cell disease is brought in because of abdominal pains; Neela takes her case while Tracy learns from her. Banfield's pregnancy test is negative, which leads to a discussion with her husband and a visit to a specialist.

Neela's day is filled with three significant events: the arrival in the ER of a man who acts out his dreams, and this time jumped through a window thinking he was escaping a plane; the deterioration of a young girl who has been her patient for a while and with whom she has a special bond, and job interviews, set up by Lucien, who also tells her he wants her to stay at county. But which version of the day is the true one? The one in which both the dream runner and the girl die because decisions turn out to be wrong, the one in which both of them die even though the decisions were right (and in which Neela blows off an interview conducted by ex-ER doc Corday)? Or the one where both patients live and she is offered the job by Corday?

And as an older woman is brought in assisted by her on-again, off-again husband, three love stories continue in the ER: Dr. Morris meets policewoman Diaz again, Sam discovers that Tony has been seeing Daria, and Neela plays it cool with the returning Dr. Brenner.

Sam's sister Kelly visits and tries to get Sam to agree to take on the care of their sick mother. A man who has lived most of his life in an iron lung needs an operation and a poor epileptic woman is brought in after shoplifting.
